Provision of Janitorial Services along with detail:-1. Complete company profile including name, registered office address, telephone, fax and e-mail address and web address (if any), complete contact details of the contact person, details of branch offices. - 2. NTN, GST & PRA registration certificates. - 3. Last 3 years Audited Financial Statements or Bank Certificates/Statements. - 4. List of Clients of similar projects. 5 projects = 5 10 projects = 7 More than 10 = 10 5. Date of establishment of business and relevant business experience of at least for 3 years. 5 years = 5 7 years = 7 More than 7 = 10 6. Details of established office setup in Lahore. - 7. Bid Security amounting to Rs.125,000/- in the form of pay order/demand draft issued by a Scheduled Bank of Pakistan, in favor of “Director General Pakistan Revenues, Lahore”. - 8. Signed cover letter with official stamp affixed on it as per the format given in Annex-I. - 9. Authority Letter for the appointment of an authorized representative as per Annex-VI. - 10. Duly filled in and signed & stamped Annex-IV: Price Schedule. Proposals not following the given format shall lead to the rejection of bid. - 11. Signed Annex-V “Compliance certificate / undertaking” - 12. Undertaking that the Bidder/firm and its employee(s) have never been blacklisted by any government, semi-government, autonomous or state owned organization and their cases regarding black listing are not under trial by any Court of Law -
